General Price List – Fees for Services Provided by our Funeral Home. State law requires that our Price list must be provided to you prior to discussing any services. We have provided it here for your convenience or you may request one from our associates prior to your appointment.

Release of Remains – Document necessary to take your loved one into our care. Must be signed by the Next of Kin that has the authority to make decisions on the loved one’s behalf.

Embalming Authorization – Form must be completed by Next of Kin that has the authority to make decisions on the loved one’s behalf. Embalming is not required but may be necessary depending on the services requested by the Next of Kin or Family members.

Obituary Worksheet– Form to assist you in writing your own personal obituary for your loved one.

Consumer Guide – State law requires that a Consumer Guide to Funeral Services be provided to every customer, free of charge, that contacts a funeral establishment prior to discussing fees or prices. We are making this available to you here for your convenience or you may request on from our representative.

Pre-Need Consumer Guide– A free guide provided by the California Cemetery and Funeral Bureau to help guide you to the best course of action for your future funeral.
